Why do I need to sign the waiver?

Before any medical procedure (treatment) you are entitled to a verbal information consultation with the treating physician.
If you wish, you may also waive this informational consultation.
In this case the physicians are obligated to document that you waived this consultation. This requires the signed waiver. Therefore please bring the signed document to your examination appointment. You may also be provided with a waiver to sign on site.
