What should I bring to the examination?

Please bring your health insurance card to your appointment. Please also remember to bring the signed waiver for the information consultation if you do not wish to have the information consultation. If it was not included with the invitation, please complete the form regarding pre-existing conditions/medical history. Of course all information is subject to medical confidentiality.
